Duplicate Document Fees
In accordance with the Regulation of the Minister of National Education of 28 May 2010 on school certificates, state diplomas, and other official school documents (Journal of Laws 2010 No. 97, item 624, as amended), and the Act of 16 November 2006 on stamp duty (Journal of Laws 2006 No. 225, item 1635).
To request a duplicate student ID card, please provide:
A written request (available at the school office)
A current, labeled photo
Proof of payment in the amount of 9.00 PLN
To request a duplicate school certificate, please provide:
A written request (available at the school office)
Proof of payment in the amount of 26.00 PLN
