Fees for Duplicates
In accordance with the Regulation of the Minister of National Education of 28 May 2010 on certificates, state diplomas and other school documents (Journal of Laws of 2010 No. 97, item 624, as amended) and the Act of 16 November 2006 on stamp duty (Journal of Laws of 2006 No. 225, item 1635), the following fees apply for issuing duplicates: duplicate student ID card: PLN 9.00, duplicate school certificate: PLN 26.00.
To issue a duplicate student ID card, please provide:
- a written application (available at the school office)
- a current, described photo
- proof of payment in the amount of PLN 9.00
